When liquid soaks through ordinary carpet padding it reaches the subfloor and stays there. You clean the carpet, the visible stain disappears, and a few days later the smell comes back — because the source was never in the carpet. It was in the plywood.
This is why so many carpets are replaced years early. The carpet is fine; the subfloor beneath it is not, and nothing you do to the surface fixes it.
